Ayushman Vay Vandana at Two Years: Who Above 70 Gets ₹5 Lakh Free Cover — and Why Most Eligible Seniors Still Haven’t Enrolled
Every Indian aged 70 or above can get free hospital cover of up to ₹5 lakh a year under Ayushman Bharat, whatever their income. Two years in, admissions are rising fast but card sign-ups lag far behind the 6 crore eligible.
Your father turned 72 this year. He has a pension, a small savings account, and no health insurance because private insurers either refused him or quoted premiums he would not pay. A cataract operation is due, and a cardiologist has mentioned the word “angioplasty”. Since October 2024, he has had a right he may not know about: free hospital treatment worth up to ₹5 lakh a year at tens of thousands of empanelled hospitals, with no income test and no medical check. All he needs is a card that takes minutes to create with an Aadhaar number.
Two years of free care for India’s oldest citizens
Prime Minister Narendra Modi launched the extension on 29 October 2024. On its second anniversary, the National Health Authority said it had authorised 23.46 lakh hospital admissions worth ₹5,668.80 crore as of 20 October 2026, according to DD News and PTI. NHA chief executive Dr Sunil Kumar Barnwal said the scheme lets seniors “access healthcare with dignity, security and peace of mind”.

- 19,483 / 16,746Public and private hospitals in that network
- 1,961Procedures in PM-JAY’s health benefit package
- 27Medical specialities covered
The hospital and package figures come from a Rajya Sabha reply by Minister of State for Health Prataprao Jadhav in March 2026, reported by Medical Dialogues. They apply to all of Ayushman Bharat PM-JAY, which Vay Vandana card holders use.
The card count that tells the real story
The scheme’s biggest gap is not money or hospitals. It is enrolment. A senior must have a Vay Vandana card to use the cover, and sign-ups, while steady, are a fraction of the eligible population.

What families get wrong: assuming that being over 70 is enough to walk into a hospital and get free treatment. Without the Vay Vandana card, the hospital’s Ayushman desk cannot authorise cashless care. Create the card before you need it; the hospital can do it at admission, but it adds stress at the worst moment.
Who qualifies, and how the ₹5 lakh is shared
The Union Cabinet approved the extension on 11 September 2024. Under the rules announced then, all Indian citizens aged 70 and above qualify, irrespective of income. Age is based on the Aadhaar record. Two features cause most of the confusion: how the cover is shared, and what happens if a senior already has other insurance.
| Your situation | Cover you get | Key rule | What to do |
|---|---|---|---|
| Single senior 70+, family not in PM-JAY | Up to ₹5 lakh a year | Cashless at empanelled hospitals | Create a Vay Vandana card |
| Couple both 70+, family not in PM-JAY | ₹5 lakh a year, shared | Cover is on a family basis | Each gets a card; the limit is common |
| Senior 70+ in a PM-JAY family | Own ₹5 lakh top-up | Separate from family’s ₹5 lakh for under-70s | Create the senior’s own card |
| CGHS, ECHS or Ayushman CAPF member | One scheme only | Must choose existing scheme or PM-JAY | Compare before switching |
| Private health insurance or ESIC | Eligible for ₹5 lakh too | Private cover does not disqualify | Keep both; use either |
| Living in West Bengal | Newly available | State signed PM-JAY MoU in June 2026 | Check local rollout status |
The shared limit matters for couples. If both spouses are 70 or older and their family is not otherwise in PM-JAY, they share the same ₹5 lakh in a year. One major procedure for one partner reduces what is left for the other.
What seniors are actually being treated for
The NHA’s two-year data shows the scheme is used mostly for the problems of ageing. Cataract surgery was the most common treatment, with 4.71 lakh procedures, about 20.1% of all authorised treatments. Chronic haemodialysis came second with about 2.90 lakh. Coronary angioplasty, known as PTCA, accounted for the most money, at about ₹649 crore.

Other leading categories named by the NHA include total knee replacement, acute flare-ups of chronic obstructive pulmonary disease, acute fever, and acute kidney injury and renal failure. Pre-existing conditions are covered from the first day, according to scheme descriptions, which is a major difference from private insurance for older buyers.
Faster growth in the second year
Usage accelerated sharply. Government data to 31 December 2025, reported by Angel One, showed 10.33 lakh admissions worth ₹2,154.37 crore. By 20 October 2026, both figures had more than doubled. The average value per admission also rose, from about ₹20,855 to about ₹24,164, our calculation.

Men used more of the cover than women
The end-2025 data showed a clear gender gap: 6.40 lakh admissions and ₹1,333.94 crore for men against 3.93 lakh admissions and ₹820.42 crore for women. The average spent per admission was almost identical at about ₹20,840 for men and ₹20,880 for women, our calculation, which suggests the gap is in access rather than in the cost of treatment.

How the ₹5 lakh stretches in practice
A worked example. At the October 2026 average of about ₹24,164 per authorised admission, ₹5 lakh covers roughly 20 average admissions in a year, our calculation. Most seniors will never come close to the limit.
The limit bites with big procedures and with couples sharing a cover. Suppose a 73-year-old husband needs an angioplasty and later a knee replacement in the same year, while his 71-year-old wife needs cataract surgery. If the family is not otherwise covered by PM-JAY, all three procedures draw from one ₹5 lakh. A private policy or a family’s savings may still be needed as a second layer.
Keep the private policy if you have one. Holding private insurance or ESIC does not disqualify a senior. Using the free ₹5 lakh at an empanelled hospital can protect the private policy’s no-claim bonus and limit for treatments at hospitals outside the Ayushman network.
Where the free cover stops
Vay Vandana is generous, but it is not a blank cheque. Knowing its limits before an emergency avoids painful surprises at the hospital billing desk.
Only empanelled hospitals, only listed packages
Cashless treatment works only at hospitals empanelled under PM-JAY, and the hospital is paid a fixed package rate for each listed procedure. Many large private hospitals in metros are empanelled for some specialities but not others. Before a planned procedure, ask the hospital’s Ayushman Mitra desk to confirm that both the hospital and the specific treatment are covered under the senior’s card.
Hospital stays, not routine doctor visits
PM-JAY is built for secondary and tertiary hospital care. It covers medicines and diagnostics for a short period before and after an admission, commonly cited as 3 days before and 15 days after, but it does not pay for routine outpatient consultations or long-term medicines taken at home. For many seniors, those monthly costs for diabetes, blood pressure or heart conditions add up to more than occasional hospital bills.
If a hospital asks for payment
For a covered package at an empanelled hospital, the senior should not be asked to pay for the treatment. If that happens, call the 14555 helpline or raise a grievance through the PM-JAY portal, and keep copies of every bill and receipt. Families report that disputes are easier to resolve when raised while the patient is still admitted.
Should a 70-year-old still buy private insurance? IRDAI removed the upper age limit for new health policies from 1 April 2024, and GST on individual policies has been zero since 22 September 2025. But premiums at this age are high and co-payments common. For many families, Vay Vandana as the first layer, plus savings set aside for non-network hospitals and outpatient costs, is the more practical combination.
