HDFC Ergo vs Care vs Aditya Birla vs Niva Bupa: Which Health Insurer Pays Claims and Treats Customers Best in 2026?
Four big names, five measurable tests: claim settlement, complaints, payout ratio, hospital network and plan design, using IRDAI and company data up to FY 2025-26.
You have narrowed it down to four brochures. Each one promises unlimited restoration, no room rent cap and a cashless network of thousands of hospitals. Each one has a glowing advert. The plans start to blur, and the question you actually care about is simple: when someone in your family is admitted at two in the morning, which of these companies is most likely to pay without a fight?
Brochures cannot answer that. Data can, at least partly. Insurers in India must publish claim settlement figures, the regulator publishes how much of each company’s premium goes back out as claims, and complaint counts are disclosed per 10,000 claims. Put those side by side and a clearer picture forms than any advert offers. It is not a perfect picture, and this article is explicit about where the numbers stop.
What We Know
- On FY 2025-26 claim settlement ratios compiled from insurer disclosures, HDFC Ergo scored 98.19%, Aditya Birla 97.27%, Care 96.99% and Niva Bupa 94.44%.
- IRDAI’s 2024-25 annual report puts the incurred claim ratio at 84.85% for HDFC Ergo’s health business, 71.50% for Aditya Birla, 64.53% for Care and 61.22% for Niva Bupa.
- The standalone health insurer average incurred claim ratio for 2024-25 was 68.06%, according to the same report.
- Niva Bupa reported 32% premium growth to ₹2,150 crore in April to June 2026 and an 11.1% share of retail health.
- All four sell plans with no room rent cap at the top end, 30-day initial waits and pre-existing disease waits of no more than three years, the ceiling IRDAI set in 2024.
What Is Still Unclear
- No source publishes how fast each insurer settles cashless claims at discharge, the delay patients feel most.
- Claim settlement ratios count claims, not rupees, so they do not reveal how much of each bill was deducted as non-payable.
- Hospital network counts come from insurer websites, are not audited, and change through the year.
- HDFC Ergo’s figures are for the health portion of a general insurer, which includes group and government business, so they are not a perfect like-for-like match with the three standalone health insurers.
The Four Contenders by Size, Before Any Scores
Scale matters in health insurance for one practical reason: a bigger insurer tends to have more hospital agreements and more experience negotiating package rates. It does not guarantee better service, and the numbers below show why. The largest of the four by premium is also the one with the most complaints.
The growth story is just as telling. Aditya Birla Health went from ₹1,021 crore of premium in 2021-22 to ₹6,238 crore in 2025-26, more than six times larger in four years. Care grew about two and a half times over the same period, and Niva Bupa tripled. HDFC Ergo, whose health premium was the largest of the four in 2021-22 at ₹4,941 crore, has grown more slowly and has been overtaken by both Care and Niva Bupa.
Round One: Who Says Yes to the Most Claims?
The claim settlement ratio is the share of claims an insurer settled out of all the claims it decided in a year. It is the number every comparison site leads with, and on this measure HDFC Ergo comes out ahead in each of the last three years. In 2025-26 it settled 98.19% of claims, against 97.27% for Aditya Birla, 96.99% for Care and 94.44% for Niva Bupa.
The gap between first and last looks small at under four percentage points. Turn it around and it is not. A 94.44% settlement ratio means about 5.6 claims in every 100 were not settled; at 98.19% the figure is about 1.8. On that crude reading, a Niva Bupa claim was roughly three times as likely to end unsettled as an HDFC Ergo claim in that year.
Care’s quiet climb from the bottom
The single most striking trend in the data belongs to Care. Its claim settlement ratio was 87.03% in 2021-22, the weakest of the four. By 2024-25 it had reached 96.74% and in 2025-26 it was 96.99%, a climb of almost ten points in four years. Niva Bupa has also improved, from 90.78% to 94.44%, but it started lower and remains last. Niva Bupa’s own results presentation for April to June 2026 gave a figure of 95.6%, which suggests the improvement is continuing.
A rising ratio is good news, but buyers should read it as a direction rather than a promise. Insurers can improve the figure by settling more claims, and also by deciding fewer borderline cases within the year. The complaint data, in the next round, is a useful cross-check on whether the improvement is being felt by policyholders.
Round Two: The Complaint Count Most Buyers Never See
If claim settlement shows what insurers decided, complaints show how customers felt about it. The disclosed measure is complaints per 10,000 claims, and the spread among these four is far wider than on any other test. In 2025-26 HDFC Ergo recorded 4.99 complaints per 10,000 claims. Aditya Birla recorded 18, Niva Bupa 29.19 and Care 42.
That means Care drew more than eight complaints for every one that HDFC Ergo drew, relative to the number of claims each handled. Care’s complaint rate has also risen over time, from 22.21 in 2020-21 to 47 in 2024-25 before easing to 42. Its claim settlement ratio improved over the same years, which is worth noticing. More claims are being paid, yet more policyholders are unhappy, and partial deductions or slow cashless approval are plausible reasons that the settlement ratio would not capture.
Niva Bupa’s figure deserves a fair reading too. It fell from 43.44 in 2024-25 to 29.19 in 2025-26, a drop of about a third in one year. HDFC Ergo’s rate also swung, from 14.72 to 4.99, which is a reminder that a single year can be noisy. The three-year averages smooth this out: 8.87 for HDFC Ergo, 18.67 for Aditya Birla, 37.13 for Niva Bupa and 42.67 for Care.
Round Three: How Much of Your Premium Comes Back as Claims
The incurred claim ratio, published by IRDAI, compares the claims an insurer incurred in a year with the premium it earned. A ratio of 70% means that for every ₹100 of premium, about ₹70 went towards claims. It is often misread as a measure of generosity, and it is not quite that, but it does indicate how much room an insurer has to pay out.
Two points matter. Care and Niva Bupa sit below the standalone health insurer average of 68.06%, while Aditya Birla sits above it. And HDFC Ergo’s figure is not strictly comparable, because a general insurer’s health book includes group policies for employers, which typically run at higher claim ratios than retail plans. A low ratio is not automatically bad either. It can reflect a younger, newer customer base, as is common for fast-growing insurers whose policies have not yet aged into heavy claims.
Round Four: Hospital Networks and the Cashless Promise
A cashless claim works only at a network hospital. Each insurer states its network on its own website, and these counts are useful mainly to spot very large gaps. Aditya Birla Health claims more than 16,500 hospitals, HDFC Ergo more than 16,000, Care more than 11,400 and Niva Bupa more than 10,000. Niva Bupa’s results presentation for the first quarter of 2026-27 gave more than 10,600.
The national total matters far less than the hospitals within 10 kilometres of your home. A network of 10,600 that includes the two hospitals you would actually use beats a network of 16,500 that does not. Every insurer’s website has a hospital locator; checking your three nearest preferred hospitals takes about five minutes and is the most useful step in this whole comparison.
Since IRDAI’s master circular of 29 May 2024, every insurer must decide a cashless request within one hour and approve final discharge within three hours of the hospital’s request. Those rules apply equally to all four, so the remaining difference lies in how often each insurer meets them. That is precisely the figure none of them publishes.
Round Five: Inside the Four Flagship Plans
Service data tells you how an insurer behaves. The plan wording tells you what it has promised. Each of these companies sells a premium flagship, and the four have converged on most basics: no room rent cap in the main variant, a 30-day initial waiting period, two years for specified illnesses and three years for pre-existing conditions. The differences lie in how cover grows, how chronic illness is handled and how flexible the plan is.
| Feature | HDFC Ergo Optima Secure | Care Supreme | Aditya Birla Activ One Max | Niva Bupa ReAssure 3.0 |
|---|---|---|---|---|
| Sum insured range | ₹5 lakh to ₹2 crore | ₹5 lakh to ₹1 crore | ₹5 lakh to ₹2 crore | From ₹5 lakh, by variant |
| Cover growth | Marketed as up to 4X base cover | Bonus of 50% a year up to 100%; add-on up to 500% | Super Credit of 100% a year up to 500%, max ₹3 crore | Booster+ carries forward unused cover |
| Pre-existing disease wait | 3 years | 3 years | 3 years; 0 days for 7 chronic conditions with add-on | 3 years; can be cut to 2 or 1 year |
| Pre and post hospital days | 60 and 180 | 60 and 180 | 90 and 180 | 60 and 180 |
| Restoration | 100%, unlimited | 100%, unlimited | Unlimited reload | Unlimited ReAssure benefit |
| Distinctive feature | Large cover from day one | Low entry sum insured, simple design | HealthReturns of up to 100% of premium | Lock the Clock entry-age premium |
Plan details are drawn from the insurers’ published wordings and from comparison listings, and they change with each product revision. Treat the table as a starting point, then read the current policy wording for the exact variant you are quoted.
Aditya Birla’s edge for chronic illness
For a buyer who already has diabetes, hypertension, asthma, high cholesterol, obesity, COPD or heart disease, Aditya Birla’s Chronic Care add-on is the most distinctive feature in this group. It removes the three-year wait for those listed conditions, covering them from the first day. The other three plans can shorten waiting periods through add-ons, but Activ One’s chronic cover goes furthest. HealthReturns, which pays back up to 100% of the premium as rewards for tracked healthy activity, appeals to the disciplined; one user review notes that it requires consistent use of the app.
Niva Bupa’s flexibility, and its trade-off
ReAssure 3.0 is the most adjustable of the four. Its Lock the Clock feature charges premium based on your age at entry until you make your first claim, which can matter a great deal to a 30-year-old buying for the long term. It lets buyers shorten the pre-existing disease wait and choose an optional co-payment of 10% to 50% to reduce the premium. That flexibility is real. The question for a buyer is whether it outweighs the weakest settlement ratio in this group.
The Scoreboard: Five Tests, One Table
The table below ranks the four insurers on each of the five measurable tests. Each first place earns 4 points, second 3, third 2 and fourth 1. This is our own scoring, not an official rating, and it deliberately gives equal weight to each test. Plan features are not scored, because the right feature depends on who is buying.
| Test | HDFC Ergo | Care | Aditya Birla | Niva Bupa |
|---|---|---|---|---|
| Claim settlement, FY26 | 98.19% 4 | 96.99% 2 | 97.27% 3 | 94.44% 1 |
| Complaints per 10,000, FY26 | 4.99 4 | 42 1 | 18 3 | 29.19 2 |
| Incurred claim ratio, FY25 | 84.85% 4 | 64.53% 2 | 71.50% 3 | 61.22% 1 |
| Stated hospital network | 16,000+ 3 | 11,400+ 2 | 16,500+ 4 | 10,600+ 1 |
| Premium scale, FY26 | ₹7,152 cr 2 | ₹10,031 cr 4 | ₹6,238 cr 1 | ₹8,586 cr 3 |
| Total out of 20 | 17 | 11 | 14 | 8 |
Change the weights and the order moves. If you care only about claims and complaints, HDFC Ergo is clearly ahead and Aditya Birla is clearly second. If you treat scale as the main signal of stability, Care rises. If you are 28 and buying for 30 years, Niva Bupa’s entry-age premium may matter more than any ratio here. The scoreboard is a lens, not a verdict.
Matching the Insurer to the Buyer
The more useful question is not which insurer is best, but which is best for a particular household. The matrix below sets out how each insurer fits five common buyer profiles, based on the data and plan features above.
| Buyer profile | HDFC Ergo | Care | Aditya Birla | Niva Bupa |
|---|---|---|---|---|
| Young single, age 25 to 32 | Strong | Good on price | Good if active | Strong, entry-age lock |
| Family with children | Strong | Good | Strong | Good |
| Existing diabetes or hypertension | Good | Fair | Strongest, day-1 add-on | Good, shorter wait option |
| Parents aged 60 plus | Strong on service | Fair, check complaints | Good | Fair |
| Wants ₹1 crore or more | Strong, up to ₹2 crore | Up to ₹1 crore | Strong, up to ₹2 crore | Up to ₹1 crore |
A Twenty-Minute Way to Choose
A sound decision does not need a week of reading. These four stages, done in order, settle most choices.
- Set your sum insured first. In a metro, ₹10 lakh is a sensible floor for an individual and ₹15 lakh to ₹25 lakh for a family, before comparing insurers.
- Disclose every existing condition. Non-disclosure is among the commonest reasons claims are rejected, at any insurer.
- Ask for the variant name in writing. Each flagship has several variants, and features such as no room rent cap may apply only to some.
- Use the 30-day free-look period. If the policy wording differs from what you were told, you can cancel for a refund after small deductions.
Rule Changes That Levelled the Field
Several regulatory changes since 2024 have narrowed the differences between these insurers, because they now apply to everyone. Knowing them stops a salesperson presenting a legal requirement as a special feature.
- 1 April 2024IRDAI removes the upper age limit for buying health cover and caps the pre-existing disease wait at 3 years, down from 4.
- 1 April 2024The moratorium period, after which a claim cannot be contested for non-disclosure except fraud, falls from 8 years to 5.
- 29 May 2024Master circular requires cashless decisions within 1 hour and discharge approval within 3 hours.
- November 2024Niva Bupa lists on the stock exchanges, making it the second listed standalone health insurer after Star Health.
- 22 September 2025GST on individual health insurance premiums is removed, lowering the cost of all four insurers’ retail plans by the former 18% tax.
- September to October 2026IRDAI consults on commission caps, and insurers float a 10% co-pay proposal that has not been approved.
The five-year moratorium deserves emphasis. Once a policy has been held continuously for five years, the insurer cannot reject a claim on grounds of non-disclosure unless fraud is proved. That makes staying with one insurer more valuable over time, and makes the first choice more important than many buyers assume. Porting remains possible, and waiting periods already served carry over to the new insurer for the earlier sum insured.
Before You Sign: The Checklist
Whichever of the four you choose, these checks apply. Most claim disputes trace back to one of them.

- Complaints below 20 per 10,000 claims, ideally falling
- At least 2 of your 3 preferred hospitals in the cashless network
- No room rent cap, or a cap of at least 1% of sum insured a day
- No co-payment for your age band unless you chose it for a lower premium
- Consumables cover, worth 5% to 10% of a typical bill, included or added
- Pre-existing disease wait of 3 years or less, with reduction options known
- All existing conditions and medicines declared on the proposal form
Frequently Asked Questions
Which is better, HDFC Ergo or Niva Bupa?
On the published service data, HDFC Ergo is ahead. Its claim settlement ratio was 98.19% in 2025-26 against Niva Bupa’s 94.44%, and it recorded 4.99 complaints per 10,000 claims against 29.19. Niva Bupa’s ReAssure 3.0 is more flexible, with an entry-age premium lock, so it can suit young long-term buyers.
Is Care Health Insurance good for claim settlement?
Care’s claim settlement ratio has improved sharply, from 87.03% in 2021-22 to 96.99% in 2025-26. However, it recorded 42 complaints per 10,000 claims in 2025-26, the highest of the four insurers compared here, so check reviews and hospital tie-ups in your city before buying.
Is Aditya Birla Health Insurance good for diabetes patients?
It is the strongest of the four for listed chronic conditions. The Chronic Care add-on on Activ One covers diabetes, hypertension, asthma, high cholesterol, obesity, COPD and some heart conditions from day one instead of after the usual three-year wait. Confirm eligibility and the added premium for your case.
Which health insurer has the highest claim settlement ratio in India?
Among these four, HDFC Ergo had the highest ratio in 2025-26 at 98.19%. Across the whole market, rankings vary by year and by data source, and some smaller insurers report higher figures. Always compare at least three years, not a single year.
What is a good incurred claim ratio for health insurance?
There is no official ideal, but a range of roughly 60% to 90% is generally seen as healthy. Below that, an insurer may be paying out little; above 100%, it is paying more in claims than it earns in premium, which may not be sustainable. The standalone health insurer average for 2024-25 was 68.06%.
Is HDFC Ergo a standalone health insurer?
No. HDFC Ergo is a general insurer that sells motor, travel and home cover as well as health. Care Health, Aditya Birla Health and Niva Bupa are standalone health insurers. This matters when comparing claim ratios, because a general insurer’s health figures include different types of business.
Can I switch from one of these insurers to another without losing waiting periods?
Yes. Under portability rules you can move at renewal, and the waiting periods you have already served carry over for the sum insured you held. Apply to the new insurer before your renewal date, and do not let the old policy lapse while the application is assessed.
Is GST charged on health insurance premiums in 2026?
No, not on individual policies. GST on individual health and life insurance premiums was removed from 22 September 2025. Group health policies bought by employers are treated differently, so check your invoice if your cover comes through work.
The Short Version
If you want the insurer with the strongest service record, the data favours HDFC Ergo: the highest claim settlement ratio among the four and by far the fewest complaints. Aditya Birla Health is a strong second, with the largest stated hospital network and the best cover for chronic illness. Care Health is the biggest by premium and has improved its settlement record sharply, but its complaint rate is the highest. Niva Bupa is growing fastest and offers the most flexible plan, yet still sits last on claim settlement. Check your hospitals, check three years of data, then compare price.
